Nursing homes are rated from 1-5 stars based on health inspections, staffing, and quality measures. According to CMS, a rating of five stars means that quality is “much above average.”

Not a nursing home, it is a rehab/ physical therapy center/after care. My mom was in the hospital for 2 weeks, then came here to try and get her up and moving again. Staff treated her well… there was a sitter outside her room but looking after a few rooms. Rooms are clean and spacious, the facility is huge so they have a lot of people to look after. Since my mom needed monitoring, she was in a different wing. Staff is patient but stern. I felt bad for the staff when they would report to me that my mom hit them, but they would tell me she’s just frustrated and they are trained to handle those situations with grace and dignity. Meals are served on time, they have activities, a nice courtyard, eating area, families are welcome to visit and join some activities or just be with their loved one. If there were any changes with my mom, I was always informed. Thank you to all the staff at Canyon Vista!
The care my mother has received has been very good. The facility is nicely maintained and most staff a very friendly. The one thing that needs improvement is communication with family. A contact person should be identified upon admission. I have felt like I have to try and chase people down to get updates. Relying on my mom to report what’s happening with her care has been challenging. Therfore, direct staff contact would be helpful.
